AFLAC Open Enrollment

Dear TPOAM Members, 

Aflac Open Enrollment is going on now through September 13th. Applications and information can be obtained through Renee Lewis or at the following Court Houses:

  • 63rd District Court, 1950 E Beltline Ave NE, Grand Rapids, MI 49525, (616) 632-7770
  • 61st District Court, 180 Ottawa Ave NW, Grand Rapids, MI 49503, (616) 632-5700

All applications must be submitted by 9/16/2019 for a 10/1/2019 effective date and to meet Group Minimum Participation Requirements. Please let Renee Lewis know if you’d like to schedule an in-person or phone appointment off-site. Aflac has not been granted On-Site Enrollment Conditions this year.

Eligibility

Per the signed Master Application we have on file, the eligibility for all products except Short Term Disability is 40 hours per week and 60 days of employment. Eligibility for Short Term Disability is 40 hours per week. Seasonal and temporary employees are not eligible.

Please note: this is when the employee is eligible to apply, not the effective date.  The effective date is always on or after the enrollment form date.

 Underwriting Rules and Concessions

  • Products were last offered to new hires: 9/1/2018.
  • A new hire is anyone hired after 7/1/2018.

Critical Illness 

Guaranteed Issue:

  • Newly Eligible Employees: $20,000 for employees and $10,000 for the spouse.
    • A new hire is anyone hired after 7/1/2018.
  • Current insureds: additional buy-up benefit of $20,000 for employees and $10,000 for the spouse.
  • Late enrollees (employees/spouses who were previously eligible and elected not to enroll): $20,000 for employees and $10,000 for the spouse.
  • Maximum benefit of $50,000 for employees and $25,000 for the spouse.

Accident

Guaranteed Issue: 

The Accident plan is guaranteed issue – no underwriting is required to qualify for coverage.

Supplemental Hospital Indemnity

Guaranteed Issue:

  • Newly eligible employees are guaranteed issue.
  • Late enrollees are required to answer health questions.
  • The Hospital Admission Benefit amount for this group: $1,000.

Short-Term Disability

Guaranteed Issue:

  • Newly Eligible Employees: the guaranteed issue amount for the group risk Choice is $3,000 per month.
    • A new hire is anyone hired after 7/1/2018.
  • Late enrollees required to answer Health Questions.
  • Current insureds increases in coverage will require underwriting/health questions to be answered.
  • Maximum Income Replacement Percentage: the maximum income replacement is 60% of salary. The maximum income replacement for employees who reside in states with state disability (NY, NJ, HI, CA, RI and PR.) benefits is 40%.
  • The minimum benefit amount is $300, and the maximum benefit amount is $6,000.

Remember, the deadline for Aflac Open Enrollment ends on Friday, September 13th. If needed, please enroll by the deadline for a October 1 effective date.
